I understand that my submission may be edited for clarity, length and/or style. I consent to the collection and use of my personal information as described, in accordance with the Privacy and Data Protection Act 2014 (Vic.) I confirm that I am the parent/guardian of the minor(s) featured in the submission (story and/or photograph) and give complete consent to their inclusion. I acknowledge that I have the rights to the submitted story and photographs and grant the Haemophilia Foundation Victoria the right to use them as described.
